Passport Application Information

This page lists the requirements for the various passport applications.

PLEASE NOTE:

  • It is mandatory that applicant for passports must translate ALL documents attached to their application into English for approval and processing
  • All classes of applicants should note that submission of request as strictly by appointment. And this has to be done using the link on their payment slip after which the appointment slip printed and attached together with the rest of their documents.

Fresh Passport Application

  • Duly completed passport application form
  • Printed confirmation of online payment slip
  • Copy of birth certificate/declaration of age (as the case may be) note: applicants born in Nigeria after commencement of National Population Commission Act of 1992 are expected to bring NPC issued birth certificate or attestation of birth
  • Copy of certificate of state of origin/local government identification letter (required for adult applicants)
  • Two (2) passport-sized photographs of the applicant
  • Duly completed guarantor’s attached with passport photograph and copy of data page of Guarantor’s Nigerian passport
  • In the case of underaged applicant, letter of consent from both parents attached with copies of data page of Nigerian parents as provided in the Guarantor’s form. Note: in case of single parent, he/she must provide evidence to that effect

Extension Of Validity Of Passport That Has Not Expired

  • Duly completed passport application form
  • Printed confirmation of online payment slip
  • Data page of the passport
  • Passport booklet for sighting
  • Two (2) Passport photographs
  • Formal letter addressed to the Comptroller General of Nigeria Immigration Service and duly signed by the applicant for grant of approval of the subject matter. The letter should explain/state reasons

Re-issue Due To Expiration

  • Duly completed passport application form
  • Printed confirmation of online payment slip
  • Data page of the passport
  • Two (2) Passport photographs
  • Passport booklet for sighting

Re-issue Due To Loss

  • Duly completed Passport application Form
  • Printed confirmation of online payment slip
  • Data page of the passport (if available)
  • Extract from Police station diary bearing the passport number (Police report of the loss of passport)
  • Two (2) Passport photographs

Re-issue Due To Damage

  • Duly completed passport application form
  • Printed confirmation of online payment slip
  • Data page of the passport
  • Two (2) Passport photographs
  • Damaged passport

Re-issue Due To Marriage/divorce

  • Duly completed Passport application Form
  • Printed confirmation of online payment slip
  • Formal letter/request to change of surname (stating reason) addressed to the Comptroller General of Nigeria Immigration Service and duly signed by the applicant.
  • Data page of the passport
  • Copy of residence permit, evidence of second relationship
  • Copy of marriage certificate (original for sighting)
  • Copies of Affidavit for change of surname and copy of newspaper publication
  • Data page of spouse’s passport

Re-issue Due To Change Or Correction Of Data

  •  Duly completed passport application form
  • Printed confirmation of online payment slip
  • Formal application/request for subject matter addressed to the Comptroller General of Nigeria Immigration Service and duly signed by the applicant or parent/guardian as the case may be.
  • Data page of the passport and passport booklet submitted
  • Submission of supporting documents in respect of the request including residence permit, or evidence of second nationality as the case may be
